Owen Health Care offers health services that are specially designed and personalized to meet the needs of those we serve, and the level of our health care services contribute to the physical, psychological, and social well being of all our clients.
These are some of the services we provide at Owen Health Care:
- Skilled Nursing Care: Our RN will assess and develop a comprehensive plan of care tailored to meet your individual needs, administering medications, monitoring vital signs, and treatment for medical conditions, all under the supervision of a physician
- Clinical Social Worker: Provides social work assessments and follow-up services, including referral to resources available to you and your family.
- Behavior Specialist: Conducts functional behavioral assessments, writes positive behavior plans and provides ongoing behavioral supports.
- Personal Care Aide/Individual Support Worker: provides a variety of services under the direction of our clinical supervisors, such as implementing behavior plans, teaching social skills, parenting skills, home training, emotional supports, and life skills needed to successfully participate in the community
- Certified Home Health Aides: Provides a different variety of services under the supervision of our clinical director, including assistance with daily living activities/personal care, bathing, grooming, dressing, meal preparation, feeding, diet and nutritional reminders, health monitoring/medication reminders, assistance with ambulation/transfers and to help maintain a safe environment. Companion, shopping, errands, household, chores/light housekeeping, laundry/linen changes, and arrange/escort to appointments.
- Support Coordination: Our Support Coordination program is a Medicaid funded program for DDD eligible individuals. We provide support and linkage to services to individuals living in their own homes or with their families.
