Owen Health Care is an approved provider with the Division of Developmental Disabilities (DDD) to provide quality services to individuals with Developmental and Intellectual Disabilities. Some of our services include Support Coordination, Respite, Vocational Habilitation, Supported Employment, and Individual Supports. Our goal is to provide the individuals we serve with the tools that are necessary for their success. Our licensed clinicians and highly qualified professionals are trained to provide the highest quality of compassionate care and to promote independence to all individuals served.

Our highly experienced Support Coordinators are committed to enriching the lives of individuals through support and collaboration to ensure that support coordination services are delivered efficiently and in a timely manner based on the individual’s needs. Currently, we serve Bergen, Essex, Hudson, Somerset, Middlesex, Monmouth, Morris, and Passaic Counties. We value and adopt a person-centered and strength-based approach to deliver outcomes to individuals throughout the planning process.

Planning Process & Access to Services

  • Individuals will be assigned a Support Coordinator (SC).
  • The SC will complete a Person-Centered Planning Tool (PCPT) with the individual, their family, and team to gather information and identify strengths.
  • The SC will coordinate and facilitate a team meeting to identify service needs and develop the Individual Service Plan (ISP).
  • The SC will explore and link individuals to the appropriate services.
  • Waiver Assurance Coordinators (DDD staff) will approve the plan, authorize the service, and provide quality assurance.
  • The SC will provide quality case management and maintain phone and face-to-face contact on a minimum of a monthly basis with the individual, their support team, and providers to ensure that the ISP and services are being delivered appropriately.
